Insurance
All patients must show their insurance ID card at each visit.
Anyone requiring payment arrangement may request to speak with Mrs. Nacca, the Director, to set up a payment plan. All copays are due at time of visit.
Payment is expected at the time of each visit for services not covered by insurance.
All patients are taken as to the urgency of their situation.
We will not bill or take insurances pending. Anyone with insurance pending is to pay for the service. We will not bill an insurance, Medicare or Medicaid if not in full active state at the time of service. Payment plan can be arranged with payment down.
